Anti-GABA-B encephalitis

Anti-GABA-B encephalitis is an autoimmune encephalitis with multisystemic involvement.

It can present with psychiatric features such as acute confusion, behavioural change, agitation, anxiety, mood symptoms, sleep disturbance, hallucinations, and psychosis, particularly in the context of limbic encephalitis.

Other characteristic features include limbic encephalitis with prominent seizures (often frequent or refractory), rapidly progressive memory impairment, cognitive decline, altered mental status, and encephalopathy. Cerebellar ataxia may occur in some cases. There is a strong association with underlying malignancy, particularly small-cell lung cancer.

From a multisystemic lens perspective, clinical features in other body systems include (but are not limited to):

  • Respiratory: association with small-cell lung cancer, which may present with respiratory symptoms or tumour-related effects.

  • Renal/electrolytes: hyponatremia may occur due to SIADH, particularly in the context of associated small-cell lung cancer.

Early diagnosis is important to enable prompt immunotherapy and treatment of any underlying malignancy, reduce the risk of persistent cognitive impairment, recurrent or refractory seizures, and functional decline, and improve neurological recovery, prognosis, and quality of life.
