Long philtrum

This page focuses on long philtrum as a morphologic feature of conditions that may also be associated with psychiatric manifestations, such as psychosis, mania, autism spectrum disorder, ADHD, OCD, and self-injurious behaviours. Of note, to optimize clinical utility, the list does not include conditions associated with intellectual disability in the absence of other psychiatric features.

Long philtrum

In these images, the philtrum is increased in absolute length. However, subjective assessment requires consideration of its length relative to the length of the nose.

Definition

Subjective: Apparently increased distance between nasal base and midline upper lip vermilion border.

Objective: Distance between nasal base and midline upper lip vermilion border more than 2 SD above the mean.

Note: The length of the philtrum usually shows a relation with the length of the nose. The philtrum and nose together occupy the central face zone; a change of length of one will usually be accompanied by a compensatory, opposite change in the length of the other. Therefore a long philtrum often accompanies a Short nose, but this is not always the case and should be assessed separately.
