Widely spaced eyes

This page focuses on widely spaced eyes as a morphologic feature of conditions that may also be associated with psychiatric manifestations, such as psychosis, mania, autism spectrum disorder, ADHD, OCD, and self-injurious behaviours. The page begins with a clinical definition of widely spaced eyes based on the standardized Elements of Morphology terminology, together with visual examples from the same source. This is followed by a list of conditions in which widely spaced eyes have been described in association with psychiatric manifestations.

Of note, to optimize clinical utility, the list does not include conditions associated with intellectual disability in the absence of other psychiatric features.

Definition

Subjective: The interpupillary distance appears to be increased.

Note: Although an objective definition is available based on population mean values and percentiles, it applies only to individuals up to 15 years of age. The finding of widely spaced eyes should only be made according to the subjective definition in persons above 15 years of age. It is important to distinguish between truly increased interpupillary distance and the apparently increased spacing that is caused by Telecanthus.
